The petitioner is mother and natural guardian of disabled child namely Aashi. She claims that her daughter is suffering from Locomotor Disability/Cerebral Palsy with Quadriparesis and has been assessed with 75% permanent disability by competent medical authority. She is possessing a valid Disability Certificate and UDID Card bearing No.HR6030520060005397 issued by Civil Surgeon, Faridabad.
